In the 1970s and 1980s, medical professionals started using a method called inspiratory muscle strength training to wean people off respirators and to increase the lung capacity of people that had lung diseases.

In the first study, Colorado researchers split 13 volunteers, all with resting systolic blood pressure that was above 120 mmHg, into a real treatment group and a sham treatment group.

 

The real treatment group had to breathe through this device for six days a week for six weeks, taking 30 breaths per day. The device was set on 75 percent maximum inspiratory pressure.

 

The sham group was asked to do it for the same amount of time and number of breaths, but the device they used was a sham device that provided no resistance.

 

After six weeks, they found two important improvements in the treatment group.

 

  1. Their resting systolic blood pressure was considerably lower than that of the sham treatment group. This was just over 10 mmHg lower than prior to the study.

 

  1. Their large arteries functioned better by being better able to dilate in response to blood flow.

 

The scientists were thus pleasantly surprised, as a 10-mmHg blood pressure reduction is better than what any long aerobic exercise session can accomplish.

 

It is also better than what any single blood pressure drug can do.

 

In the second study, they asked 14 volunteers to run on a treadmill after the same six-week schedule of breathing, after dividing the volunteers into the same real and sham treatment groups.

 

After the study, the treatment group was found to be able to exercise longer, with less cardiorespiratory effort, and at a lower heart rate than the sham treatment group could.

 

The third study also used 14 volunteers divided into the same two groups, with the same breathing schedules for the same amount of time.

 

After six weeks, they again found a 10-mmHg resting systolic blood pressure reduction in the treatment group.

 

On top of that, the treatment group also showed some cognitive improvements, such as being able to process information faster and being able to remember better.

The researchers concluded that taking a patient’s blood pressure once per year (and then doing it properly) at the annual exam would eliminate the vast majority of false positives.

 

By extension, this would eliminate unnecessary follow-up tests or drug regiments.

 

Critics looking at the study results stated that the 7% error in finding actual hypertensive patients by screening only once annually was too big of an error margin and more frequent testing should be viewed as the safer route.

 

The battle underscores the need for frequent and properly conducted at-home screening. There is a growing body of evidence to suggest that home meters, when used at the recommended 3-times-per-day interval and in the proper way, are a much more accurate gauge in understanding how a patient is faring than measuring blood pressure in the stressful environment that is the doctor’s office.

 

The proper way includes making sure the person being tested is seated, has been at rest for at least 5 minutes prior to the test, and that the testing arm is supported. Both sides of the controversial study agree that this is not being done in the doctor’s office, which accounts for many of the errors.

The study tested whether medication and clinic appointment reminders via text message increased people’s compliance with their blood pressure treatment and lowered their blood pressure more than just what their prescribed treatments were expected to.

 

They recruited almost 1,300 participants from the poorer areas of Cape Town and divided them into three groups.

 

1) The first group received only their blood pressure treatment.

 

2) The second group received their treatment together with text message reminders to take their medication, attend their scheduled doctor’s appointments, do their exercises, and eat a healthy diet.

 

3) And the third group received their treatments, the reminders, and the interactive ability to cancel their doctor’s appointments and change the timing of their messages.

 

Those who received the text messages lost 2.2 mm Hg of systolic blood pressure points more than those who received only their treatments, primarily because a higher number of them took their medication 80 percent or more of the time, 61.2 versus 49.4 percent.

 

The ability to interact and cancel appointments or messages made little difference.

 

One of the authors stated that the improvement was approximately the same as what would be expected from one-on-one behavioral counseling, which is considerably more expensive.

 

Now, I’m not the world’s number one fan of blood pressure medications but I’m fascinated by this study because it can just as well be used to remind us to take natural measures.

An article by British researchers in the March 2010 edition of the Lancet medical journal concluded that people with systolic blood pressure, particularly those that varied from one doctor’s visit to the next, were at a much higher risk of stroke than those who simply had high blood pressure.

 

While the study participants all suffered from high blood pressure, those whose scores changed the most between low and high suffered the most strokes, particularly if the highest point was extremely high.

 

Remember that your systolic blood pressure is the higher of the two numbers on your blood pressure reading. It measures the pressure of the blood pumped from your heart into your arteries. If the normal blood pressure is 120 over 80, then systolic would be the 120.

 

An article published in the September 2015 edition of the Annals of Internal Medicine had a more alarming than the findings of the Lancet study. An impressive 25,814 patients with high blood pressure were subjected to repeated readings over a period of 22 months. They were subsequently monitored for almost three years, to find out who suffered the most strokes and heart attacks.

 

Those with the largest variations between readings had a 30% greater risk of suffering a heart attack, a 46% greater risk of suffering a stroke, and a 25% greater risk of suffering from heart failure.

 

And if this is not bad enough, those that had systolic readings that were higher than the 14-point variation were 58% more likely to die than those with smaller variations. In other words, if your blood pressure, for example, sometimes reads 130/90 and sometimes 150/100, you are 58% more likely to die in the next three years when compared to someone with stable high readings.

 

One thing that the researchers are not able to know yet is the direction of the causal relationship. Does highly variable blood pressure cause the cardiovascular conditions or are there other conditions already present that are causing the highly variable blood pressure.

 

This is important because, if the highly variable readings are caused by some third health condition, researchers would need to discover this deadly condition very soon.

 

If, on the other hand, the unpredictable levels are the cause of the heart attacks and strokes, researchers should develop a treatment that keeps blood pressure consistent, as opposed to just lowering it.

 

However, the bottom line is that high blood pressure needs to be managed. You need to get it down, whether it’s spiking or not.

– twice or less per week
– three or more times per week
– unknown

 

Compared to never-snorers, those in the heaviest snoring group had 4.57 mmHg higher systolic and 2.58 mmHg higher diastolic blood pressure scores.

 

Those who snored three or more times per week also had a 79 percent greater chance of having high blood pressure when compared to never-snorers.

 

These findings were true for overweight, normal-weight, and lean people.

Gradually, the standard turned into a universal cut-off level of blood pressure that was between 140/90 mmHg to 150/100 mmHg, at which a doctor would recommend a lifestyle change or medical intervention.

 

Eventually, the target was reduced to 140/90 mmHg for healthy adults and 120/80 mmHg for adults with kidney-related disease or diabetes.

 

Lower is Better

 

Recent evidence from a National Institutes of Health study has convinced medical researchers to once again lower their recommendations regarding safe blood pressure levels.

 

The long-term study, which began in 2009, is known as the Systolic Blood Pressure Intervention Trial or SPRINT, and has confirmed what many doctors have learned through years of clinical practice.

 

According to John Kostis, director of the Cardiovascular Institute at Rutgers Robert Wood Johnson Medical School, he said: “the lower the better and the earlier the better.”

 

This means that the best way to prevent strokes, heart attacks, and other cardiovascular events would be to take aggressive and swift action as soon as high blood pressure is detected, regardless of the patient’s age.

 

Focusing on systolic blood pressure – the upper number of the blood pressure ratio represents pressure in arteries during heart contraction, SPRINT researchers sought to find out whether reducing this value to a maximum of 120 mmHg would offer any benefit to adult patients. These patients would be aged 50 years or older, with hypertension and who were considered to be at risk in developing heart or kidney disease.

 

Results were overwhelmingly in favor of the lower blood pressure levels. By comparison, a systolic of 120 decreased heart attack, heart failure, and stroke rates by nearly a third and risk of blood pressure-related death by close to 25%, when compared to the standard 140-mmHg systolic pressure.

 

So, how did SPRINT researchers accomplish their goal? The standard dose of any of the five classes of blood pressure drugs produces about a 10-mmHg drop in systolic blood pressure and, statistically, doubling the dosage of a single drug would provide less than a 50% additional return.

 

When there is a large gap between actual and ideal systolic pressure, it becomes necessary to mix and match drugs and dosages. As a result, to bring their systolic pressure down to 120 mmHg, some SPRINT participants required as many as four different medications.

 

The Inevitable Controversy

 

However, not all experts agree on the cardiovascular benefits of using multiple blood pressure drugs. According to biostatistician Dr. George Howard from the University of Alabama at the Birmingham School of Public Health, your risk of stroke may even rise if you take multiple blood pressure medications. This can be an increase of as much as 33% with each additional drug.

 

This evidence opposes the SPRINT results. Additionally, with more drug prescriptions comes the increased risk for side effects and adverse drug interactions.

 

So, while SPRINT provides doctors with a protocol and rationale they can use for potentially improving the health and lives of their patients, it may not be without risks.

 

The Take-Home

 

Since hypertension is largely a preventable, lifestyle-related disease, it usually responds well to dietary and lifestyle changes. Experts continue to emphasize the need to stop smoking, get regular exercise, maintain a healthy body weight, and consume alcohol in moderation.

Researchers at Australia’s Monash University have discovered a major cause of blood pressure.

 

If they are right, it could lead to a brand new type of treatment.

What is going on here? Are active immune cells not supposed to ensure great health?

 

This is true in most cases. However, the stimuli that cause hypertension, like stress, make these B cells too active in a situation where there are no foreign organisms to attack. They then produce excessive numbers of antibodies that would attack your arteries.

 

The research team found great numbers of these antibodies inside the walls of the mice’s arteries, leading to an inflammatory response that hardened and stiffened those arteries. This increased their blood pressure because their arteries were no longer flexible enough to respond properly to blood flow.

 

This discovery is important, as it would suggest that potential treatments could focus on blocking the activity of the B cells that produce antibodies in a situation where threatening foreign organisms are absent. Drugs that do this are already prescribed for people with rheumatoid arthritis, which is caused by a similar B cell overreaction.

 

This does not free us from the responsibility of adopting a healthy lifestyle to prevent high blood pressure, of course. The treatment that blocks the B cells would likely consist of a synthetic drug with its own side effects and toxicity.
